<p>Since 2008, the MDSP, with funds provided by Novo Nordisk, has conducted a grant process to encourage senior-serving organizations around the country to promote the free diabetes screening benefit under Medicare. To date, over 40 organizations across the country have been awarded small grassroots organizing grants to spread the message of diabetes screening within local communities. This year, the MDSP is partnering with the National Senior Corps Association for the administration of the annual MDSP community grants.</p>

<p><a href="http://www.nscatogether.org/">The National Senior Corps Association</a> is a membership organization that advocates for and supports the needs of Foster Grandparent, RSVP, and Senior Companion programs nationwide. NSCA is the recognized national leadership organization representing the combined interests of the Foster Grandparent Program, RSVP, and the Senior Companion Program. The Foster Grandparent Program connects people 55 and over with children and young people with exceptional needs. RSVP offers &#8220;one stop shopping&#8221; for all volunteers 55 and over who want to find challenging, rewarding, and significant service opportunities in their local communities. The Senior Companion Program brings together volunteers age 55 and over with elderly adults and people with disabilities in their communities who need assistance to retain their independence.  Our partnership with NSCA allows the MDSP grants to continue to support a network of local senior-serving organizations with the goal of spreading the diabetes screening message.</p>
<p>NSCA and the MDSP are pleased to announce the grantees of the 2012 Medicare Diabetes Screening Project Grants.  This year, National Senior Corps Association received a large number of applications (56).  15 grants totaling $35,000 were awarded to very diverse organizations around the country.</p>

			<content:encoded><![CDATA[<p>The YMCA’s Diabetes Prevention Program is a 16-session, evidence-based lifestyle intervention shown to reduce the risk for type 2 diabetes. In a classroom setting, participants learn how to improve eating habits, increase physical activity and make other changes that can lead to a healthier lifestyle.</p>

<p>In this Atlanta-based initiative, locally-known as the <a href="http://www.getajumpondiabetes.org/">Get a Jump on Diabetes</a> campaign, Medicare-aged residents with a diagnosis of prediabetes and who are overweight are eligible to participate in the program through the YMCA of Metro Atlanta at no cost. We’d really appreciate your help spreading the word about this campaign to older adults who are at risk of developing diabetes. Here’s how you can help:</p>
<ol>
<li><strong>For older adults to be eligible,</strong><strong> they must be: </strong>1) 65 or older, 2) overweight with body mass index greater than or equal to 25 and 3) have prediabetes with a fasting blood glucose level between 100 and 125 mg/dl or anA1C level between 5.7%-6.4%.</li>
<li><strong>If older adults are eligible, they should call </strong>Linda Vaughn, YMCA of Metro Atlanta, at (404) 527-7690 to confirm their eligibility and find a class location near them.</li>
<li><strong>Please follow-up with them. </strong>Remind older adults to call Linda at the YMCA of Metro Atlanta to get enrolled. It could be a chance for them to turn their lives around.</li>
</ol>
<p>We really need everyone’s support in raising awareness about this campaign! Making these lifestyle changes might not be easy for older adults, but it is proven and possible with the YMCA’s Diabetes Prevention Program.</p>

			<content:encoded><![CDATA[<p>(Louisville,  KY)   Kentucky is among the ten states with the highest incidence of diabetes in the United States.  Ten per cent of Kentuckians are estimated to have diabetes, 20 percent of Kentuckians over the age of 65.  Clearly, the probability of getting diabetes goes up as Kentuckians get older.</p>
<p>To help combat the growing threat of diabetes, the YMCA of Greater Louisville and the YMCA of Central Kentucky in Lexington  have announced a new program that will provide scholarships to qualifying adults over the age of 65 enabling them to participate in the YMCA&#8217;s Diabetes Prevention Program.</p>
<p>The “Get a Jump On Diabetes” scholarship campaign was announced Wednesday at a meeting of diabetes awareness advocates, diabetes educators, health care providers and others actively involved in efforts to prevent diabetes in Kentucky.</p>
<p>Erin Brown, Association Wellness Coordinator for the YMCA of Greater Louisville and Jay Hedlund, National Director of the Medicare Diabetes Screening Project,  introduced the program in Louisville, which will provide scholarships valued at about $300 to the first 50 senior adults in the YMCA of Greater Louisville service area,  as well as 50 scholarships to seniors in the YMCA of Central Kentucky&#8217;s service area. In order to qualify, adults must be over the age of 65, overweight, and diagnosed with prediabetes.</p>
<p>The YMCA Diabetes Prevention Program, which is open to adults of all ages, consists of sixteen weeks of classroom study led by a lifestyle coach who helps older adults learn about improving eating habits, ways to increase physical activity, and other changes than can lead to a healthier lifestyle.  The initial 16 weeks are followed by a maintenance program of up to a year.  The goals are to reduce body weight by 7 percent and increase physical activity to 150 minutes per week.</p>
<p>“Diabetes is one of the most serious health issues facing older adults in Kentucky,” says Brown.  “Yet research has shown that older adults can reduce their risk of developing type 2 diabetes by as much as 71 percent by participating in programs that help participants lose weight and increase physical activity.”</p>
<p>To qualify for the scholarships, senior adults, age 65 and older, must be be overweight and referred by a physician or health care professional certifying that blood values meet prediabetes levels.  Qualifying individuals do not have to join the YMCA to receive the scholarship and participate.</p>
<p>The scholarships are funded by Novo Nordisk through the Medicare Diabetes Screening Project, a coalition of more than 20 national patient, provider, industry and government organizations, co-chaired by the American Diabetes Association, the Healthcare Leadership Council and Novo Nordisk.</p>

			<content:encoded><![CDATA[<p><em><strong>100 Seniors Will Receive Scholarship Funding For Their Participation in the YMCA&#8217;s Diabetes Prevention Program</strong></em></p>
<p>Maricopa Integrated Health System (MIHS) along with Valley of the Sun YMCA have announced a new and innovative collaboration with the Medicare Diabetes Screening Project through Novo Nordisk Inc., a leader in diabetes care.   MIHS will identify, recruit and refer up to 100 adults, age 65 and older, with prediabetes to YMCA’s evidence-based Diabetes Prevention Program.  The Diabetes Prevention Program, beginning Fall 2011, will be offered at MIHS facilities and local YMCAs in the greater Phoenix area.  Scholarships covering participation in the program will be funded by Novo Nordisk.</p>
<p>The burden of diabetes is enormous in older adults in America.  Nearly eight of every 10 people have either diabetes or are prediabetic.  According to the Centers for Disease Control and Prevention (CDC), approximately 27 percent of all US adults aged 65 and older have diabetes, and about one-fourth of them are undiagnosed.  In Arizona, where the rate of diabetes is 31 percent higher than the national average, it is estimated that 248,100 adults, 65 years and older, have diabetes and 461,000 more with prediabetes.</p>
<p>“In Maricopa County, we estimate more than 250,000 older adults with prediabetes.  Many of them could benefit from a diabetes prevention program that is available right in their communities,” says Patricia Stradleigh, RN, Manager of the MIHS’ El Mirage Family Health Center.  “With this program, we have the opportunity to identify appropriate English/Spanish speaking candidates for diabetes prevention and give them a chance to participate in their own backyards.”</p>
<p>Ms. Stradleigh will lead a team of Care Managers at seven of MIHS’ 11 Family Health Centers to assist in identifying participants in this program.  The criteria for eligibility include age (65 and older); overweight or obesity as defined by having a body mass index (BMI) ³ 25; a diagnosis of prediabetes, as defined by blood test values on the A1C (5.7 – 6.4%) or fasting plasma glucose (100 – 125 mg/dl) blood tests; and having an existing relationship with a primary care provider.</p>
<p>Diabetes prevention is proven and possible, especially for older adults.  The Diabetes Prevention Program, a large and powerful research study conducted by the National Institutes of Health, showed that adults with prediabetes could reduce their risk of developing Type 2 Diabetes by 58 percent through a lifestyle intervention program of relatively modest weight loss and increased physical activity.  Of special interest, older adults (60+) in this study reduced their risk by 71 percent.  The YMCA’s Diabetes Prevention Program – which is part of the CDC-led National Diabetes Prevention Program – is based on this study.</p>
<p>“We are excited at the prospect of partnering with Maricopa Integrated Health System to enroll additional older adults in the YMCA’s Diabetes Prevention Program,” said Sarah Shimchick, Director of the Valley of the Sun YMCA Diabetes Prevention Program.  “We deeply appreciate Novo Nordisk’s investment that will help us continue to support individuals in reducing their risk for Type 2 Diabetes.”</p>
<p>The first 100 eligible seniors will have their choice of attending YMCA’s Diabetes Prevention Program either onsite at the MIHS El Mirage or Glendale Family Health Centers, or local Valley of the Sun YMCA location. Transportation assistance is available.</p>

			<content:encoded><![CDATA[<p>The Medicare Diabetes Screening Project hosted  a Lunch and Learn event at Atlanta City Hall on Wednesday November 9th during National Diabetes Awareness Month. At this event, participants learned about the <em>Get a Jump on Diabetes Campaign,</em> and how to qualify for scholarships to the YMCA’s Diabetes Prevention Program that will be given to eligible adults. The event  featured a line up of community leaders discussing the program and its effectiveness in reducing the risk for Type 2 diabetes.  Attendees included City of Atlanta employees, Activity Coordinators, and seniors from the community. The MDSP also provided a healthy lunch for all attendees.</p>
<p>Through the <em> Get a Jump on Diabetes Campaign</em>, older adults with a diagnosis of prediabetes and who are overweight are eligible to participate for free in the YMCA&#8217;s Diabetes Prevention Program at Metro Atlanta YMCAs, thanks to scholarships provided by Novo Nordisk. In a classroom setting, a trained lifestyle coach helps older adults learn about improved eating habits, ways to increase physical activity and other changes that can lead to a healthier lifestyle.</p>

Atlanta’s first-of-its-kind diabetes prevention scholarship program for older adults, called <a href="http://www.getajumpondiabetes.org/"><em>Get a Jump on Diabetes</em></a>, hosted a Lunch and Learn event in DeKalb County  on World Diabetes Day, November 14<sup> th</sup>. At this event, featuring DeKalb County CEO Burell Ellis, participants learned how to qualify for scholarships to the YMCA’s Diabetes Prevention Program that will be given to eligible adults 65 or older.   The November 14<sup>th</sup> event featured a line up of community leaders including CEO of DeKalb County, Mr. Burrell Ellis,  Dr. Elizabeth Ford, M.D., DeKalb County Board of Health District Health Director, Linda Vaugh of the YMCA, and Maurice Madden, Georgia Director of the MDSP discussing the program and its effectiveness in reducing the risk for Type 2 diabetes. Free lunch for all attendees was provided by The MDSP.</p>

<p>Through this program at the YMCA, older adults with a diagnosis of prediabetes and who are overweight are eligible to participate for free in the program at Metro Atlanta YMCAs, thanks to scholarships provided by Novo Nordisk. In a classroom setting, a trained lifestyle coach helps older adults learn about improved eating habits, ways to increase physical activity and other changes that can lead to a healthier lifestyle.</p>
<p>Research has proven that older adults with prediabetes can reduce their risk of developing type 2 diabetes by as much as <strong>71 percent</strong> by participating in lifestyle modification programs, such as the YMCA’s Diabetes Prevention Program. In addition, a recent study from Emory University showed that these types of prevention programs can save more than $<strong>7 billion in Medicare costs</strong>. This new program in Atlanta will serve as a pilot program for the rest of the nation.</p>

<p>Congratulations to all of our Grant Award Winners from 2010-2011! Here are some of the highlights of their activities.</p>
<p><strong>Positive Image Productions </strong>in Missouri City, Texas is in the final stages of producing a video piece entitled, “Diabetes, Medicare and You.” The piece features information aimed at seniors about Medicare and diabetes screening. It includes a basic description of Type 2 diabetes and shows an African American woman receiving a diabetes screening test. Her doctor then discusses the symptoms of diabetes and steps that can be taken to prevent the disease.</p>
<p><strong>Family Service Upper Ohio Valley</strong> in Wheeling, West Virginia was featured on a one-hour broadcast by WWVA Radio (1170 AM) with Family Service CEO Lonnie Wineman, guest Heather Leonard, Diabetes Education Coordinator for Ohio Valley Medical Center and East Ohio Regional Hospital, and Doctor Richard Greco, D.O.  The show is called “DOCTALK” and the topic for the show was “Diabetes and the Medicare Diabetes Screening Project.” The radio coverage for WWVA, a 50K watt station, is approximately 255,800 listeners, with more than 35,800 listeners who are age 65 and older.</p>
<p><strong> </strong></p>
<p><strong>Maricopa Integrated Health System (MIHS)</strong> in Phoenix, Arizona has incorporated five diabetes screening questions into MIHS’ electronic medical records.  Providers are now prompted to ask the questions to any patient who is 65 years old or older and follow-up with a screening test if a patient is determined at risk for diabetes. Additionally, all clinic providers at MIHS have received information about the Medicare Diabetes Screening Project and instructions explaining the appropriate medical coding for diabetes screening tests covered by Medicare. MIHS reported that they reached 19, 715 Phoenix area seniors through their efforts of material dissemination, presentations and trainings to partner organizations and at health fairs, as well as local media coverage on the benefits of diabetes screening.</p>
<p><strong>The Heritage Area Agency on Aging (AAA)</strong> in Cedar Rapids, Iowa distributed 2,000 pieces of Medicare Diabetes Screening Project material to congregate and home delivered meal participants throughout a seven county region and 30 MDSP tool-kits to dining site managers. The Heritage AAA was also featured in an episode of the “Involvement Report,” a local television program that agreed to dedicate an entire segment to the Medicare Diabetes Screening Project and diabetes education.</p>
<p><strong>Alliance for Aging </strong>in Miami, Florida provided diabetes education at senior centers, as well as distributed information and resources at several health fairs in the area. Alliance for Aging also managed radio outreach to Haitians, African Americans, and Hispanics through a weekly radio show on La Poderosa and WMBM radio stations. All together, the stations reach Miami-Dade, Monroe, Broward, and Brevard counties.</p>
<p>The organization estimated that the number of seniors reached was 10,296.</p>
<p><strong>Area 1 Agency on Aging </strong>in Humboldt and Del Norte Counties, California has produced several communication tools, including tools internally such as “Gray Matters”, a quarterly publication of the organization that included an article about MDSP in the Fall 2010 edition. Members of the organization have been promoting the project through various other community-wide efforts as well.  The organization’s Executive Director, Cindy Denbo, presented information on the Medicare diabetes screening benefit  on the feature segment,  “Live at Five”, on local television. The segment itself reached 5,000 people in the area. Area 1 Agency on Aging also created two separate Public Service Announcements that were played repeatedly last fall on two local popular radio stations throughout the surrounding counties. To further inform Medicare beneficiaries about the importance of diabetes screening Deborah Krzesni, R.D., gave presentations to older adults at the Title III Elderly Nutrition Program Congregate lunches. These presentations included information on the need to be screened annually, eligibility requirements, goals for diabetes management, and principles of the diabetic diet. Members of the Title III Elderly Nutrition Home Delivered Meal Program were also provided materials and risk assessments pertaining to the early screening and prevention of diabetes.</p>
<p><strong>The Johnson City Senior Center Foundation </strong>in Johnson City, TN has reached approximately 10,800 seniors through their efforts. The Johnson City Seniors’ Center conducted a kickoff and diabetes conference with local physicians and health care professionals. Participants received education about prevention, diagnosis, and treatment of diabetes, as well as pledge cards, and one-on-one education about MDSP. There were also newspaper ads and articles announcing the project campaign kickoff and diabetes conference.</p>

<p><strong>The NCW Rural Health Foundation </strong>in Wenatchee, WA sent information regarding the MDSP to the Wenatchee Area Network for Diabetes, as well as Allen Dietz, Chief Operations Office of Community Choice Healthcare Network. The Community Choice SHIBA volunteers informed clients of the Medicare diabetes screening benefit.</p>
<p><strong>Grand Forks Parks and Recreation Foundation</strong> in Grand Forks, North Dakota has partnered with the YMCA Family Center, Grand Forks Center Court Fitness Club, and the Grand Forks Senior Citizens Center, along with Ascent Health Resources, to hold three events promoting the Medicare diabetes screening benefit. The events provided education and resources about diabetes prevention and health to seniors in attendance. Held in May of 2011, these events promoted the importance of being screened for diabetes, education on the Silver Sneakers Program, and helped to enroll those diagnosed with prediabetes into the “I CAN Prevent Diabetes” program taught by WelCore Health.</p>
<p><strong>Bristol Elder Services </strong>in Fall River, MA sent a press release to local newspapers announcing the grant from the MDSP. An article was printed in the Fall River Herald on November 11, 2010 to describe the grant.  Bristol also circulated informational flyers in the February mailing of both Bristol home care clients and meals on wheels clients. MDSP will also be discussed on the upcoming monthly radio segment.</p>
<p><strong>Carroll County Bureau of Aging and Disabilities</strong> in Carroll County, Maryland has reached approximately 1325 seniors through their efforts with the grant. The Bureau of Aging and Disabilities has done several presentations of information to community groups and at senior centers across the county. They have also disseminated MDSP materials to five public library locations, and displayed the materials at Maryland Senior Drug Assistance Program events.  MDSP materials are also available to the public at the Bureau of Aging and Disabilities offices.</p>
<p><strong>Hanul Family Alliance (Korean American Senior Center) </strong>in Chicago, Illinois has provided a diabetes self-management program for Korean American seniors with diabetes, and their caregivers.  A total of 15 seniors with diabetes, and their caregivers, attended a six-week diabetes self-management program. Hanul developed program materials, such as a program flyer, and distributed the information about the importance of diabetes screening and self-management to seniors in the area.</p>
<p><strong>COSSMA, Inc. </strong>in Cidra, Puerto Rico presented an education booth entitled, “Knowing if you have diabetes is better than not knowing,” “Saber que tienes diabetes es major que no soberlo.” The booth was exhibited at several health fairs throughout the island country. During the health fair events, all seniors age 65 and older with Medicare coverage received a card with a message in Spanish about the risk factors for diabetes and how they can be screened.</p>
<p><strong>Elder Options </strong>Between February and August of 2011, Elder Options has promoted the Medicare Diabetes Screening Project  at approximately 36 health fairs, 7 presentations, 21 evidenced-based workshops, and 93 outreach activities. The service area includes the counties of Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Putnam, Sumter, Suwannee, and Union, covering 11,000 square miles of the state of Florida. These activities held at senior centers, libraries, churches, retirement facilities, assisted living facilities, senior housing complexes, and congregate meal sites reached over 3,000 seniors throughout a 16-county service area. Most of the targeted seniors were low-income, rural, and limited English and minority populations. Information about the MDSP was also promoted at all of Elder Options community health and wellness presentations.</p>
<p><strong>Macoupin County Public Health Department </strong>The Macoupin County Public Health Department in Carlinville, IL reached over 2,000 seniors through their education efforts between February and August of 2011.  During this time, displays with the MDSP educational information were created at various health fairs, community centers, and in county offices.  Various county vehicles used for local transportation were outfitted with bright yellow magnet signs to raise awareness of diabetes screening. These vehicles are used on a daily basis, and are parked in a highly visible parking lot.</p>

The event, held at the East Lake Family YMCA, included Dominique Wilkins, former Atlanta Hawks forward and NBA Hall of Famer, as well as prominent Emory University  Professor and Executive Director of Partnership to Prevent Chronic Disease, Dr. Kenneth Thorpe. Other speakers at the event who relayed the message of the importance of diabetes screening and prevention included, Dr. Pamela Roshell, Senior State Director for AARP Georgia, Linda Vaughn, Wellness Director for the Metro Atlanta YMCA, Bill Fallin, Lifestyle Coach for the YMCA’s Diabetes Prevention Program, and Maurice Madden, Georgia Director for the Medicare Diabetes Screening Project. The event included senior attendees, MDSP coalition partners, local media outlets, and members of the community.</p>

			<content:encoded><![CDATA[<p>ATLANTA, GA (September 28, 2011) – The Medicare Diabetes Screening Project (MDSP) in collaboration with the Atlanta Regional Commission’s Area Agency on Aging (AAA) and the Metro Atlanta YMCA today launched a first in the nation diabetes scholarship to enroll adults 65 or older with prediabetes who are overweight in the YMCA’s Diabetes Prevention Program. In this Atlanta-based partnership, the YMCA’s Diabetes Prevention Program is available free of charge to 200 Atlanta-area residents 65 or older who have prediabetes and who are overweight.</p>
<p>The National Institutes of Health Diabetes Prevention Program research has shown that programs that help individuals lose a moderate amount of weight and increase their physical activity can reduce the risk of developing type 2 diabetes by 71 percent among older adults. In Georgia, approximately 794,000 older adults, or 72 percent of those 65 or older, have diabetes or prediabetes.</p>
<p>“It’s been shown that diabetes prevention is proven and possible,” said Kristin McEwen of the Metro Atlanta YMCA. “We know that losing weight and increasing physical activity is a challenge, but the YMCA’s Diabetes Prevention Program has been proven effective. We are thrilled that scholarships are now available in the Atlanta area for older adults with prediabetes to take advantage of the YMCA’s Diabetes Prevention Program.”</p>
<p>In this Atlanta-based initiative, locally-known as the <em>Get a Jump on Diabetes</em> campaign, Medicare-aged residents with a diagnosis of prediabetes and who are overweight are eligible to participate in the program at Metro Atlanta YMCAs at no cost, thanks to scholarships provided by Novo Nordisk. In a classroom setting, a trained lifestyle coach helps older adults learn about improved eating habits, ways to increase physical activity and other changes that can lead to a healthier lifestyle.</p>
<p>Among the Medicare population age 65 or older living in Atlanta, it is estimated that more than 30,000 have undiagnosed diabetes and more than 80,000 have prediabetes. Adults 65 or older should ask their doctors to be screened for diabetes to determine if they are at risk. Medicare covers free annual diabetes screenings with no co-pay and no deductible for those at risk.</p>
<p>“This disease is personal for me because my mom, aunt and I all have diabetes,” said Former Atlanta Hawks forward and Basketball Hall of Famer Dominique Wilkins, who is a spokesperson for the <em>Get a Jump on Diabetes</em> campaign. “I’ve teamed up with the MDSP for the second year in a row to spread the word to older adults in Atlanta about the importance of screening and the need to make lifestyle changes in order to prevent diabetes or delay its onset.”</p>
<p>“The good news is that diabetes can often be prevented, and those found to have it can take simple steps to control it,” said Maurice Madden, Georgia Director of the Medicare Diabetes Screening Project. “The first step is to ask a health care provider and find out if you are at risk.”</p>
