Care Improvement Plus supports the Medicare Diabetes Screening Project in order to: 1) raise awareness of the issue of diabetes among Medicare beneficiaries; 2) encourage evidence-based medicine practices, including appropriate preventative care and medication compliance; and, 3) to help beneficiaries better understand their medical conditions, such that they may make informed choices regarding their Medicare coverage.  Care Improvement Plus is a unique Medicare Advantage Part D (MA-PD) health plan exclusively designed to meet the needs of Medicare beneficiaries living with diabetes, heart failure, chronic obstructive pulmonary disease (COPD), and/or end stage renal disease (ESRD). Care Improvement Plus offers more benefits than Original Medicare and many other Medicare Advantage plans, including care management programs and in-home health monitoring.
